Supported Living Services
At The Partnership of Care, we’re proud to offer inclusive, person-centred supported living across South Wales. We support adults aged 18+ with a wide range of needs, including learning disabilities, autism, complex behaviours, physical disabilities, acquired brain injuries, and mental health conditions.
Our simple approach: We help people live as independently and happily as possible, in homes that feel like home, not services.
We currently operate 28 supported living services across the Caerphilly and Pontypool areas, as well as in Blaenau Gwent, Newport and Torfaen. These services are delivered in collaboration with local councils and Aneurin Bevan Health Board.
What we offer
Our supported living services are built around each person’s assessed needs. Whether someone needs light support with daily tasks or 24-hour care for complex behaviours, we create packages that reflect who they are, what they enjoy, and how they want to live.
We support individuals with:
- Learning disabilities and/or autism
- Physical and sensory impairments
- Acquired brain injuries
- Dementia
- Mental health conditions
- Complex behaviours that challenge
- We do not directly provide nursing care, but work in partnership with healthcare professionals to ensure holistic support.
Our Ethos
At the heart of our supported living service is a belief in dignity, empowerment and growth. We match individuals with staff based on skills and compatibility and involve families and professionals every step of the way.
We follow a person centred care and support planning process using the Nourish Care platform. Each person has an outcomes based support plan based on their assessed needs, goals and preferences, reviewed regularly and shaped by those who know them best.

Staff & Training
Our staff teams are carefully selected, well-trained and fully registered with Social Care Wales.
All support workers receive a thorough induction and complete ongoing training in areas including:
- Learning disabilities and autism
- Positive Behaviour Support (PBS)
- Active Support
- TEACCH and intensive interaction
- Trauma-informed practice
- Medication, epilepsy, dementia, and first aid
- Health & safety, safeguarding, and communication skills
We also have in-house PBM trainers and TEACCH specialists who ensure the people we support receive high-quality, consistent care.
